Before you commit.
Having out-of-network benefits is a starting point. Your plan’s requirements, deductible, coinsurance and any authorization requirements may affect coverage and what you pay.

A FEW THINGS TO KNOW
Your questions,
answered.
Are you in network with my insurance?
No. The practice is out of network. Our team can review whether your plan includes out-of-network benefits and explain the next steps.
Does having benefits mean surgery is covered?
No. Benefits verification is not a guarantee of coverage or payment. Your insurer determines coverage under your plan, and deductibles, coinsurance, exclusions and authorization requirements may apply.
Which appointments are complimentary?
The benefits check is complimentary. If the review is favorable, the consultation with our physician assistant is also complimentary. A subsequent surgeon consultation is a separate appointment; fees and potential coverage will be clarified before scheduling.
What if I don’t know what benefits I have?
You can still request a benefits check. Our team will contact you to explain what information is needed for the review.
Do you see patients from New Jersey and Connecticut?
Yes. We welcome patients from New York, New Jersey and Connecticut at our Manhattan practice. The team can discuss appointment logistics when they contact you.
INFORMATION FIRST. YOUR DECISION NEXT.
